"Mr. Yan, let me tell you whatâs really going on."
Yang Xuan walked to the bedside and reached out to smooth the messy hair of the woman on the bed.
"Mr. Yan, itâs like this..."
"Three days ago, my younger sister, Yang Ting, suddenly started speaking nonsense. Sheâd cry one moment and laugh the next, saying all sorts of incoherent, jumbled things."
"She also drools..."
Yang Xuan pulled a few tissues from the box on the nightstand, wiped the drool from her sisterâs mouth, and continued, "Sometimes, she gets agitated and starts screaming and thrashing."
"We were afraid she would hurt herself, so we had no choice but to tie her up."
Yan Feifan asked coldly, "Sheâs been like this for three days. Why have you kept her tied up at home instead of taking her to a hospital?"
Yang Xuan bit her lip, a look of shame on her face. "My parents and I were being selfish."
"Mr. Yan, my fiancé and I have set our wedding for May Day. If his family finds out my sister is like this..."
"Theyâll definitely worry that I might also..."

"Weâre planning to take her to a psychiatric hospital out of province for treatment."

Yang Xuan gritted her teeth and nodded. "Yes. My paternal grandmother has schizophrenia. Sheâs currently receiving treatment at a psychiatric hospital in the neighboring province."

The genetic probability of mental illness is quite high.
Studies show that the incidence rate of schizophrenia among relatives ranges from 4.3% to 16.4%, far higher than the 1% rate in the general population.

He reached out and touched Yang Tingâs forehead.
Yan Feifan immediately noticed that her forehead was hot. Her temperature was likely between 38.5 and 39 degrees Celsius.
He looked at Yang Xuan, his expression serious. "Did your sister have a fever before this episode began?"
"Or did the fever only appear after?"
Seeing how gravely Yan Feifan asked the question, Yang Xuan didnât dare answer carelessly. She thought about it seriously and said, "Mr. Yan, I really donât know."

Yang Xuan looked at her parents. "Do you two know?"
The Yang parents exchanged a glance.

He opened his medical bag, took out a stethoscope, a penlight, and a pair of rubber gloves, which he put on.

At Yan Feifanâs request, Cai Yuanheng and Mr. Yang left the room, while Yang Xuan and her mother stayed.
Yan Feifan first used the stethoscope to listen to the patientâs heartbeat, lung sounds, and bowel sounds. He found her heart rate was high, at one hundred and thirty beats per minute, but he didnât hear any murmurs. Her lung and bowel sounds were also normal.
Next, Yan Feifan began his examination with the patientâs head and face.
He checked her head for any signs of trauma, insect bites, or parasites. He also checked her pupillary response to light, her mouth and tongue coating, and her ear canals for inflammation.
Finding no obvious abnormalities on her head or face, Yan Feifan had Yang Xuan remove the patientâs pajamas.
After a visual inspection confirmed there were no bite marks from insects or snakes, nor any external injuries, he began the hands-on examination.
Yan Feifan felt the patientâs neck, then moved down along her collarbones, pressing and palpating her arms and ribs.
Of course, he didnât skip examining her breasts either.
Moving past her navel, his hand pressed onto the patientâs lower abdomen...

Listening to Yan Feifanâs logical explanation, Yang Xuan glanced at her sister, who was drooling profusely on the bed again, then looked back at him.
